The aim of this questionnaire is to determine all aspects for your dog's lifestyle, enabling me to pinpoint the
source of any issues and to create a plan to help improve their behaviour.
Medical History - Please list any injuries/illness. Please include any current medication/treatment.
What type of food does your dog eat and how frequent are the meals?
Any other dogs at home?
What training have you experienced so far? E.g. puppy classes, sessions with another dog trainer(s), etc.
What does your dog’s daily routine look like? Please include as much as possible i.e. wake up time, food, walk times and places, time left alone, time spent training/playing.
How many and how long are the walks your dog has? Are the walks mainly on or off lead? Please describe how your dog behaves on your walks, with as much detail as possible.
How would you describe your dog’s personality?
What incentivises your dog? E.g. food, toy, ball...
How is your dog with other dogs and people? Including elderly, children, male, female, etc.
